PROGRAM PARTICIPANT
WAIVER
In
consideration of my participation in Shalom
Hartman Institute of North America (the “Organization”)’s Hartman Leadership Conference San Francisco 2026 taking place
on 10/23/25 – 10/25/26 at The Palace Hotel (the
“Program"), and other good and valuable consideration, I agree as follows:
1. I hereby acknowledge that:
- My participation in the Program is entirely voluntary.
- I will comply with the customary rules and conditions for
participating in the Program. I will
inspect any facility and equipment to be used, and if I believe anything is
unsafe or unusual, I will immediately advise necessary personnel of such
condition(s) and refuse to participate.
- I am not
taking, and will not take during the Program, any drugs that are
contraindicated for the Program, or I have received a physician’s approval
to participate in the Program while taking such medication. If necessary, I have received advance
approval from any treating physician to participate in the Program. If needed, I have disclosed in advance
any accommodation I may need in order to participate in the Program.
- I am of
lawful age and legally competent to sign this waiver.
2. I acknowledge that in connection with the Program, I may engage in
activities such as travel that may involve potential risk of serious injury and
illness. I hereby assume all risks in connection with my participation in
the Program, for any harm, injury, illness or damage that I may suffer, both
foreseen or unforeseen. In addition, I,
for myself, and my heirs, executors, administrators, trustees, legal
representatives, successors and assigns (the “Releasors”) hereby fully release
and discharge the Organization, and its direct and indirect parents, subsidiaries,
affiliates, together with their respective officers, directors, partners,
employees, agents, members, trustees, Board members, attorneys, successors and
assigns (collectively, the “Releasees”) from any and all claims, relating to,
or arising out of, my participation in the Program, including without limitation
claims for personal injury or illness and/or other tort-like causes of action.
3. The terms of this agreement
and all rights and obligations of the parties thereto, including its
enforcement, shall be interpreted and governed by the laws of New York, without
regard to principles of conflict of law.
