PHOTO RELEASE FOR USE OF INDIVIDUAL NAME AND LIKENESS 

 

 

 

By entering an event associated with "The Shalom Hartman Institute of North America" (“Event”) you acknowledge and agree that you may be filmed, videotaped and/or photographed in connection with the filming, videotaping and/or photographing of the event by "The Shalom Hartman Institute of North America" (“Company”), its agency and/or either of their authorized representative(s). You hereby acknowledge and agree, to being included in film, video, photography and or other recording, relevant to the Event and grant Company, and to other persons as Company may designate from time to time, the absolute right and permission to include your appearance and to use your name, image, personas, photographs, pictures, voices, statements, biographical information, or other likenesses, in whole or in part, throughout the world, in perpetuity for any legal purpose whatsoever, including advertising, promoting and marketing the Event and/or Company and its products and services, in any and all media now existing or hereafter created. You hereby waive all rights to inspect and approve the finished product of any and all such materials, as well as the use thereof or such visual, written or audio copy as may be used in connection therewith. You agree not to hold Company or any of its representatives responsible for any claim or liability resulting from the use of any film, video, photography or other recording in which you appear. If you do not so agree to the foregoing, you are responsible for notifying Company prior to entering the Event, or otherwise you are not authorized to enter the Event. 

 

You represent and warrant that you are over 18 years of age and have the right to make this agreement and grant the rights granted herein, and that if you are not over the age of 18, your parent or legal guardian has agreed to the foregoing and is signing this agreement on your behalf.  You agree that this agreement will be construed and enforced in accordance with the law of the state of New York without regard to choice of law and/or conflict of law principles.
